If you are a new patient to our practice, please arrive 15 minutes prior to your scheduled appointment time. This will allow time for checking in and new patient paperwork to be completed. If you have internet access, we encourage you to print new patient forms from patient information section of our website. This will allow you to print, fill out the required documents at home, and bring them with you. This will eliminate the need to arrive early for your appointment. We can also send a link to your personal e-mail address upon request to fill out most of the paperwork online.

We need you to bring the following:
□ Insurance cards
□ Driver’s license or ID card
□ Referral for your visit (if applicable)
□ Medication list
□ Medical imaging on CD and reports performed of the body part being evaluated (X-rays, MRI, CT, etc).

The office is open Monday 8:00 AM to 5:00 PM, Tuesday 9:00 AM to 3:00PM, Wednesday 8:00 AM to 5:00 PM, Thursday 9:00 AM to 3:00PM, Friday 9:00 AM to 2:00 PM. We are closed on Saturdays and Sundays. For emergency situations, our answering service is available 24 hours a day, 7 days a week.

Our office is located at 1129 Merrillville Rd, Crown Point IN 46307. The office building is south of the Jewel Osco/BMV shopping complex and is directly east of Pappas restaurant. We are in the same building as Sweat 219 Yoga and 413 Dance Academy whose entrances are seen from Merrillville Rd. We are located on the north side of our building complex. There is parking directly in front of the office entrance. Please enter the office through the main door which leads to the patient waiting room. Do not enter through the door marked EXIT ONLY.

There is handicap parking and handicap accessible ramp available immediately in front of our office entrance. We do have a wheelchair in our office that is available for patient use. Please note we are an outpatient facility and do not have medical staff lifting assistance available for patient care. If medical staff lifting assistance is required for the patient to be seen (ie patient is unable to transfer from car to wheelchair or wheelchair to xray table or chair), please contact EMS or medical transport service to arrange transportation to your appointment.

For convenience, we have an online appointment link on our website home page to schedule both new patient and return visit appointments. Please click the link or click here to set up an appointment. Most insurance plans allow an individual to be seen by a orthopedic surgeon without a referral. If you are in an EPO or HMO plan, you may need a referral from your family physician or primary care provider. If you are unsure if a referral is necessary, please contact our office at 219-213-6630 so we can verify your insurance and let you know if any additional information is needed prior to your appointment.

If you have concerns or questions regarding your treatment plan, upcoming surgery, or medication, you can contact our office at 219-213-6630. The office staff have been trained to answer many questions and will communicate information to the appropriate medical staff as necessary.

Please contact our office directly if you need a medication refill. Do not contact your pharmacy for any refill requests. In addition, it is our policy to not renew medications, even for chronic ailments, if the patient has not been evaluated within the year. Due to the increasing need for prior authorizations, medications will be only refilled during clinic hours and may need additional time to process the refill. No medications will be refilled after hours, weekends, or holidays. Please refer to the prescription policy handout given at your appointment for further information.

If you require a form to be filled out for insurance reasons, short-term or long-term disability, worker’s compensation, FMLA or any form that is manually produced, there is a flat fee of $30 per form which is payable in advance. We will only fill out forms regarding musculoskeletal issues that Dr. Bayer has addressed in the office. Please allow 3-7 business days for completion of your form.

If you need to cancel your appointment, please notify the office at least 24 hours prior to your scheduled time. Please be advised that we charge a $35 fee for not showing up for a scheduled appointment. This fee is required due to the increasing number of patients not showing up for their appointments, which unfortunately reduces the available appointments and delays care for our other valued patients.

A deductible is the dollar amount that must be paid by the insured (ie, patient) before their insurance policy will start paying for the covered expenses. A patient is responsible for allowable charges until the deductible is met.
A co-payment (co-pay) or co-insurance is the dollar amount the insured (ie, patient) is responsible for paying for each visit. This dollar amount may be noted on the front of your insurance card. Advanced Orthopedics will collect your co-payment at the time of your service. Once your maximum out of pocket for the year has been met, you are no longer responsible for your co-payment or co-insurance.
Maximum out of pocket is the most you to pay for covered services in a year. This includes the amount spent on deductibles, co-payments, and charges from covered medical services. Once this amount has occurred, your insurance pays 100% of the cost of the covered benefits.
